Simplifying wound management services through a supported shared care pathway

Research shows that helping patients to share the care of their wounds improves self confidence and enhances the understanding of their condition. It also improves Quality of Life and provides more convenient care. 

What do we mean by simple wounds?

• Superficial or 0-2cm in depth
• No undermining
• No evidence of tunnelling
• Wound is not infected or deteriorating

The Supported Shared Care Patient Journal should be used by Healthcare Professionals as part of your patient assessment.  Patients should also use this to keep a log on their wound progression, reporting back any issues where necessary.
